We are seeking a motivated and experienced individual to drive the development of our children’s young persons service contract for the provision of family and Community Support for children with severe and complex disabilities and/or autism, living in their own homes.
This includes an outcomes-based approach working with Children and Young People towards increasing self-esteem, enhancing life opportunities and support leading to independent living and support managing basic everyday living tasks, in transition to adulthood.
You will be mainly office based 8am - 4pm Monday - Friday (Worthing West Sussex) However the role will require spending time with the children/young persons in their own homes.
- Manage and oversee the implementation of children’s young peoples care services according to the service contract with West Sussex County Council
- Carry out risk assessment’s (including manual handling or medication assessment(s), where appropriate) at the start of the service. This will identify how to manage risks and be used to influence and guide care and support planning. The aim of risk management will be to maximise the Children and young peoples independence and choice whilst maintaining and promoting personal safety.
- Recruit and line manage support staff
- Supporting our support staff to ensure each individual child has the best possible outcome.
- Work closely with the Council’s Children and Disability Services in order to meet the needs of Children and Young People
- Be part of organisational out of hours rota for on call staff.
- Maintain professionalism at all times in relation to working practices, communication, dress, punctuality and attendance
- Maintain all recording and reporting management systems as required
- Assist in developing and improving quality to enhance services as required
- Understand the principles of, and have the skills to effectively implement, Positive Behaviour Support (PBS);
- Trained and experienced in communication techniques, such as Makaton, Picture Exchange Communication System (PECS) or others as required
- Trained and experienced in Percutaneous Endoscopic Gastrostomy (PEG)/Enteral Tube Care.
- Requires in-depth knowledge and experience of Safeguarding procedures
