We are based in Stoke On Trent but you would be required to travel to neighbouring counties for Asssessments, meetings and spot checks.


Full job description


This is a full time position and a care coordinator is required to lead teams daily, create weekly staff rotas, staff training, care plans, risk assessments and other duties required for the day to day running and growth of the organisation.

Experience needed.


Potential candidates must have great interpersonal skills, a flexible approach and able to complete tasks to a high standard. They will need to be skilled at building relationships and inspiring others to achieve.


They are required to have a good knowledge and experience in care.


They should be skilled at dealing with conflicts and challenging situations, using tact, fairness and diplomacy .


They should be able to motivate your team and create an enjoyable but professional work environment.


Responsibilities:


- Coordinate and schedule tasks required for the day to day running of the company.

- Maintain accurate records of client information .

- Communicate effectively with clients, other professionals, and team members.

- Assist in resolving client enquiries and concerns in a timely manner.

- Collaborate with multidisciplinary team to ensure holistic and seamless care for clients.

- Create rotas weekly ensuring all shifts are covered at all times.

- Support our on call system when rostered to do so. Answer calls politely and respectfully.

- Take part in client assessments, creating care plans and reviewing care needs.

- Carry out supervision and spot checks of the care team

- Audit and monitor systems which are already in place ensuring high standards are maintained at all times.

- Assist the Registered Manager with the recruitment, induction, training and supervision of Staff.

- To ensure good understanding of safeguarding for adult and the channels of reporting. To ensure high standard of care is delivered across the board at all times. To carry out internal audit and ensure services are in compliance with all CQC regulations. To work with the manager to ensure quarterly audit and compliance using appropriate organisation compliance tool.

Qualifications /Experience:


- NVQ level 2 or 3 in health and social care preferred. - Office / Administrative experience - Strong organizational skills - Excellent communication skills - Ability to multitask and prioritize tasks effectively - Proficiency in Microsoft Office - Attention to detail and accuracy in data entry - Ability to work independently and as part of a team

- Driving Licence (required)
