Monday to Friday and on call at weekends


Experience in domiciliary care and coordination of services


Duties:

 

•! Work well with clients, their families and other members of the team.

 

•! Create and agree a written individually tailored plan of care and support that respects the client’s wishes and promotes their dignity and privacy. Agree appropriate risk control measures to reduce identified risks. Keep care and support plans up to date.

 

•! Provide the client and where appropriate their representatives with information about the service so that they are clear about what to expect and how they can raise any concerns

 

•! Keep all information about client’s and their families secure and confidential.

 

•! Work well with staff, clients and their relatives.

 

•! Give care workers appropriate information and guidance to effectively and safely provide care and support as agreed in the care and support plan.

 

•! Work with the Assistant Manager to appropriately match care workers to clients. Attend the client’s first call to introduce the care worker. Report changes to the office administrator/ Assistant Manager.

•! Distribute, collect and monitor Medication Administration Record (MAR) charts.

 

•! Provide advice, training, supervision and other support to Care Workers. Liaising with other supervisors as required and when required.

 

•! To provide and record at least quarterly ‘one to one’ supervision for all Care Support Workers.

 

•! To report to the office Assistant Manager and office Administrator any missed calls.

•! Take part in the out of hour’s emergency on-call rota as and when required. Effectively deal with emergency on - call issues, for example covering calls either with another Care Support Worker or covering the care calls personally, when Care Support Workers are sick or absent.

•! Record emergency calls on Quik plan and pass the information to appropriate parties.

 

•! To carry out spot checks to service users homes whilst Care Support Workers are in attendance at least 6 monthly and record on the Quik plan system.

 

•! To participate in work shadowing and the integration of Care Support Workers into their new environment.

 

•! To liaise with management in supervising or assessing new members of staff during their initial induction and shadowing period.

 

•! To meet with new staff every week for 12 weeks to assist with completion of their induction and care certificate and feeding back any concerns any concerns to the Assistant Manager.



Essential skills/Experience Level 2/3 Health & Social Care Full UK driving licence
