We are looking for an experienced nurse to take on the role of Senior Clinical Lead, managing a small team Clinical Leads across the region. 
 
We have an opportunity to join us as a Senior Clinical Lead Nurse (equivalent to a band 8a). You will oversee the packages in your area based on clients’ needs and plan your time accordingly. You will be accountable for a small team of Clinical Lead Nurses, who are responsible for leading, supporting and facilitating the nursing and medical needs of our clients and their care. You will also use your experience to work with the broader organisation to ensure quality and compliance standards are maintained.
 
The role can be hybrid as there maybe a need to travel on occasion to visit your team members.
 
Principle accountabilities:
 
• Delivering personalised client-centred service outcomes, carrying out care assessments, writing care plans, and managing competencies
• Managing a team with a coaching approach, ensuring the team are engaged and motivated with the training and tools to be a well performing, collaborative team
• Supporting the Head of Clinical Services and the Registered Manager to further develop and add strength to our clinical governance and the services we provide into the community
• Maintaining your professional skills and undertaking required training and development for your own personal professional growth, with a motivation to cascade key learning to the wider team
• Performing in accordance with national standards, legislative requirements, relevant regulations, and accepted best practices
 
You’ll have the opportunity to:
 
• Develop individualised care plans in partnership with the client, family, and other healthcare professionals, ensuring goals are patient-focused, realistic, and achievable
• Monitor client and family-centred outcomes, using your expertise to guide and implement changes proactively
• Undertake initial holistic nursing assessments in partnership with the service user, family, and carer, involving other statutory/voluntary/third-party sector providers where necessary
• Oversee and manage the regional care delivery team daily, working with committed carer teams to maintain all required standards
• Act as a responsive escalation, providing support and advice to the clinical team for them to deliver their role successfully within the care packages to the highest quality
• Ensure all recording systems and organizational documentation are of a high standard and up to date
• Motivate and manage your nursing/care team, embedding the company’s values and behaviours and fostering a culture of ‘pride’ in their work
• Manage and support field-based care support workers and nurses through regular supervisions, competency assessments, and appraisals
• Report against targets and standards; implement action plans for service improvements that positively impact clients, service quality, and business development
• Commit strongly to quality standards to ensure compliance with statutory, mandatory, and contractual requirements
Working with the broader organisation to ensure any issues are addressed and changes implemented
 
Working with others:
 
• Build meaningful and positive relationships with internal teams, understand internal frameworks to enable successful cross collaboration with Talent, HR, and Operations to ensure packages are well resourced and managed.
• Motivate carers and nurses through informal check-ins, formal supervision, and appraisals with the support of HR and peers within Sonderwell
• Working with the Care Coordination team have oversight of the rotas, ensuring that field staff are appropriately trained, assessed, and signed off as competent before lone working
• Promote and protect the welfare of our service users.
• Work in cooperation with members of the multidisciplinary teams to maximize opportunities for our service users, and identify information, concerns, and ideas for improvement and share them with the senior management team of
Libertatem
 
 
A bit about you:
This role is ideal for someone looking to blend clinical nursing skills with coaching and management of people and services. Experience working in the community is preferred. You should have experience with Spinal Injury, Acquired Brain Injuries and Cerebral Palsy and be able to pass on your knowledge to others. You should be comfortable working with both adults and paediatric care; we will provide training where required.
 
• Professional nursing registration with no restrictions, working at an equivalent to band 8c
• Relevant clinical experience – ABI/Spinal Injury/Community Nursing
• Proven ability to develop effective working relationships with key stakeholders such as: Case Managers, Integrated Care Systems, General Practitioners, Consultants and other Healthcare Professionals within the multi-disciplinary team
• Experienced with line management skills, with a motivation to develop and coach well-performing teams
• An excellent leader, who improves capability, and exhibits an engaging and adaptable style that connects with people at all levels
• Demonstrable experience in clinical governance and quality improvement activity, with a clear commitment to quality standards to ensure compliance to statutory, mandatory and contractual requirements
• Solid experience of incident and complaint management
• Passion to deliver outstanding care in a rewarding and sometimes emotionally challenging environment• Proficient IT skills
• Driving Licence
